You will complete the following segments. Please look at the list below to see who does what.

When you are ready to begin click on the begin link. Read the website carefully and answer all questions. Go through the website filling in the blanks, once you have completed the blanks printout your answers and staple the printout to the back of this document.

When you have finished go back to the main site skip questions 3 and 4.

Go to question 5 printout the worksheet and answer the questions.

Make sure all printouts are stapled to the back of this sheet and turned into my box.
